A Status- The employer must pay at least half of the premiums for the employee with the employee responsible for the balance. The employee’s portion cannot exceed 1.5% of their gross monthly wages.
B Status- The employer’s contribution toward the family premiums must equal the required single rate contribution plus half the difference between the single rate and family rates.
Total Number of Eligible Employees (Active full time employees working a minimum of 20 hours per week for four consecutive weeks.)
